NOTE: Majority of forms are reviewed by the Ethics Commission during the closed portion of their monthly meeting. In order for your form to be placed on the next agenda, forms must be submitted by close of business the Friday prior. All forms submitted after the fact will be reviewed at the following month's meeting. Individuals can expect an official response from the commission within two weeks after their review.
